(For the purpose of non – emergency medical consultation only केवल गैर–आपातकालीन चिकित्सा परामर्श के उद्देश्य से)

We are committed to give maximum possible care to all our patients, particularly in this difficult time of CORONA pandemic. As per govt advisory, we are trying our best to keep social distancing and proper sanitization at our clinic / centre. We are working with minimum possible staff. We seek co-operation from our patients in the pursuit of continuing our highly ethical & safe medical practice. हम अपने सभी रोगियों की अधिकतम संभव देखभाल करने के लिए प्रतिबद्ध हैं, खासकर कोरोना महामारी के इस कठिन समय में। सरकार की सलाह के अनुसार, हम अपने क्लिनिक / केंद्र में सोशल डिस्टन्सिंग और उचित स्वच्छता रखने की पूरी कोशिश कर रहे हैं। हम न्यूनतम संभव कर्मचारियों के साथ काम कर रहे हैं। हम अपनी अत्यधिक नैतिक और सुरक्षित चिकित्सा पद्धति को जारी रखने के लिए अपने रोगियों से सहयोग चाहते हैं।

We request our patients to follow the under written protocol strictly, without any exception. हम अपने रोगियों से अनुरोध करते हैं कि वे बिना किसी अपवाद के निम्नलिखित प्रोटोकॉल का कड़ाई से पालन करें।

1. Please stay at home and stay safe. कृपया घर पर रहें और सुरक्षित रहें।

2. Please visit our clinic / centre for medical consultation, only if it is very important. कृपया चिकित्सीय परामर्श के लिए हमारे क्लिनिक / केंद्र पर तभी आए, जब यह बहुत महत्वपूर्ण हो।

3. We are trying to avoid crowding at our clinic. So please try to maintain punctuality for the given appointment time. Prior appointment is mandatory. Walk – in patients shall not be allowed. We are trying our best to keep your waiting time minimal, but can not assure for the same due to following reason: a) Earlier patient if comes late. b) If earlier patient spends more than expected time with doctor due to various reasons. c) If doctor is expected to attend some unexpected emergency. हम अपने क्लिनिक में भीड़ से बचने की कोशिश कर रहे हैं। इसलिए कृपया दिए गए समय के लिए, समय की पाबंदी बनाए रखने का प्रयास करें। पूर्व अपॉइंटमेंट अनिवार्य है। वॉक – इन रोगियों को अनुमति नहीं दी जाएगी। हम आपके प्रतीक्षा समय को कम से कम रखने के लिए अपनी पूरी कोशिश कर रहे हैं, लेकिन निम्नलिखित कारणों से आश्वासन नहीं दे सकते हैं: क) यदि पिछला रोगी देर से आता है। ख) यदि पिछला रोगी किसी कारण से डॉक्टर के साथ अपेक्षित समय से अधिक समय लेता है। ग) यदि डॉक्टर को कुछ अप्रत्याशित आपातकाल में किसी और मरीज को देखना होता है।

4. It is mandatory for everyone (patient as well as attendant / family member) to wear standard face mask / face cover all the time, while present in the clinic /centre / waiting area / doctor’s chamber etc.. We may provide 3 ply face mask to the needy, free of cost, if required. If you are using “mask with valve”, then it is mandatory to cover it by second face mask. Your face mask / face cover must cover your chin, mouth & nose properly. यह हर किसी के लिए अनिवार्य है कि (मरीज के साथ–साथ परिचारक / परिवार के सदस्य), क्लिनिक / सेंटर / वेटिंग एरिया / डॉक्टर के चैंबर आदि में हर समय 3 प्लाई फेस मास्क / फेस कवर पहने। हम जरूरतमंदों को जरूरत पड़ने पर नि: शुल्क, 3 प्लाई फेस मास्क प्रदान कर सकते हैं। यदि आप “वाल्व वाला मास्क” का उपयोग कर रहे हैं, तो इसे दूसरे फेस मास्क द्वारा कवर करना अनिवार्य है। आपका फेस मास्क / फेस कवर आपके मुंह, ठोड़ी और नाक को अच्छी तरह से ढंकना चाहिए।

5. Hand sanitization is mandatory for everybody (patient as well as attendant / family member) before entering the clinic / centre. Please carry a small pack of hand sanitizer with you. We provide hand sanitizer to all visitors free of cost, if required. क्लिनिक / केंद्र में प्रवेश करने से पहले हर किसी के लिए (मरीज के साथ–साथ परिचारक / परिवार के सदस्य) हाथ की सफाई अनिवार्य है । कृपया अपने साथ हैंड सैनिटाइज़र का एक छोटा पैक लेकर चलें। जरूरत पड़ने पर, हम सभी आगंतुकों को हैंड सैनिटाइजेशन नि: शुल्क प्रदान करते हैं।

6. Consultation charges must be paid digitally at the time of taking appointment. अपॉइंटमेंट लेते समय ही, परामर्श शुल्क का भुगतान डिजिटल रूप से किया जाना चाहिए।

7. All patients are requested to visit with maximum one attendant only to avoid unnecessary gathering. Only, patient, will be allowed inside the doctor’s chamber. Though, attendant can accompany the patient, if there is any problem with patient mobility or conversation. सभी रोगियों से अनुरोध किया जाता है कि वे अनावश्यक भीड़ से बचने के लिए अधिकतम एक परिचारक के साथ आएं। केवल, रोगी, को डॉक्टर के कक्ष के अंदर जाने की अनुमति दी जाएगी। हालांकि, रोगी के चलने या बात करने में कोई समस्या होने पर, रोगी के साथ अटेंडेंट भी जा सकता है।

8. Every patient is expected to fill corona consent form immediately after reaching the clinic. Please do not visit:

   if you or your attendant / accompanying person have suffered from any ILI (influenza like illness) ie fever, cough, cold, difficulty breathing, any other COVID symptom in the last 21 days. Please ensure. If any of these symptoms are present, you shall be returned and referred to appropriate doctor / medical facility.

   Or if you or your attendant / accompanying person were under quarantine / isolation / being a contact of COVID patient anytime in last 21 days. हर मरीज को क्लिनिक / सेंटर पहुंचने के तुरंत बाद कोरोना सहमति फॉर्म भरना अनिवार्य है। यदि आप पिछले 21 दिनों में किसी भी ILI ( इन्फ्लूएंजा जैसी बीमारी) यानी बुखार, खांसी, जुकाम, सांस लेने में कठिनाई, किसी अन्य COVID लक्षण से पीड़ित रहे हैं तो, 21 दिन की लक्षण विहीन अवधि पूरी होने पर ही आएं। कृपया सुनिश्चित करें। यदि इनमें से कोई भी लक्षण मौजूद हैं, तो आपको लौटा दिया जाएगा और उपयुक्त चिकित्सक / चिकित्सा सुविधा के लिए भेजा जाएगा। कृपया ना आए, अगर आप पिछले 21 दिन में कभी भी आइसोलेशन में या क्वॉरेंटाइन में रहे हैं या किसी कोविड-19 के मरीज के संपर्क में आए हैं तो |

9. It is mandatory for every visitor to have “Aarogya setu app”, downloaded & active in his / her respective mobile phones. प्रत्येक आगंतुक को अपने – अपने मोबाइल फोन में “आरोग्य सेतु ऐप“, डाउनलोड करना और सक्रिय होना अनिवार्य है।

You can talk to our doctors and nursing staff regarding how to take care for patient at home. Consultations on phone and home visit can be done.
